About Your Plan

Community Care Health provides a variety of HMO plans, in addition to Dental, Vision and Pharmacy coverage. Because we work closely with your employer to tailor a plan that works specifically to meet your needs, we do not take a one-size-fits-all approach.

For plan details, including breadth of coverage, co-pays, deductibles, premiums, etc., we encourage all members to speak with their Human Resources Department or whomever is in charge of benefits administration. You can also login to the CCH Member Portal to learn more about coverage and benefits.

How Your Plan Works

As an HMO, CCH provides access to a network of more than 2,100 providers, including 1,200+ physicians (primary care and specialists) and 400+ mental health practitioners, practicing at more than 700 sites and offering urgent care at 17+ locations.

As the name implies, your Primary Care Provider will be your primary point of care.

If a specialist is needed, your PCP will make a referral to an in-network provider to minimize out-of-pocket costs. If CCH does not have an in-network specialist, however, we will cover out-of-network costs as detailed in your plan coverage.

If you choose to go out-of-network when an in-network provider does exist, you will be responsible for its costs as detailed in your plan coverage.
