All plans include hospitalization, outpatient services, prescription drug coverage and no out-of-pocket cost preventive services.

Large Group Plan
We make it easy to find the plan you need from a full range of traditional HMO plans to High Deductible Health Plans (HDHPs).
Community Care Health offers plan options that allow you to tailor your benefit needs to your business needs, choosing what you feel is most important in your health plan.
Our plans include Vineyard, Orchard and Harvest HMOs as well as the Summit HMO HDHP and the Glacier HMO/HSA COMP. Additionally, CCH has several other plans to choose from and can customize a plan to work best for your needs. Detailed information about each is available in the Large Group Brochure.
For your convenience, CCH provides multiple lines of coverage in one place. CCH Large Groups are offered medical + dental + vision or medical with either dental or vision. When bundling more than one line of coverage, discounts may apply.
